A condition is imposed for an audit to be completed on Part 3 of Schedule 1, Provider Governance and Operational Management, within 6 months of the Provider?s registration coming into force. A condition has been imposed, for providers of 0107 Assistance with daily personal activities, for risk assessment of participants who choose to be supported by a single worker. A condition is imposed for assessment of the remaining elements of Module 5 to be conducted 3 months after service delivery has commenced in class of support 0131. A condition is imposed for assessment of the remaining elements of Module 2a to be conducted 3 months after service delivery has commenced on 2a Implementing Provider. A condition is imposed for assessment of the remaining elements of the Certification audit to be conducted 3 months after service delivery has commenced in any of the classes of support that require certification assessment. A variation condition is imposed for the assessment of Module 5A: Assistance with SIL. The audit must be conducted as part of the provider?s mid-term audit which must commence no later than 18mths after the date of registration.
